Agkistrodon contortrix snake venom, commonly known as the copperhead, Agkistrodon Contortrix is a pit viper found throughout eastern and central North America. It is one of the most frequently encountered venomous snakes in the United States. Bites are relatively common, but fatalities are rare due to the venom’s moderate potency and the availability of medical care.

Copperhead venom is primarily hemotoxic, with a complex mixture of enzymes and proteins. Key components include metalloproteinases, serine proteases, phospholipase A2, and L-amino acid oxidases.

Metalloproteinases degrade extracellular matrix proteins, contributing to local tissue damage, edema, and hemorrhage. Serine proteases interfere with the coagulation cascade, causing coagulopathy and prolonged clotting times. Phospholipase A2 enzymes disrupt cell membranes, leading to myotoxicity and inflammation. The venom also contains small amounts of neurotoxins, but neurotoxic effects are uncommon in human envenomation.

Bites typically produce immediate pain, swelling, and erythema at the site. Local tissue damage can be significant, with blistering and ecchymosis developing over several hours. Systemic symptoms are usually mild and may include nausea, dizziness, and mild coagulopathy. Severe complications such as compartment syndrome, necrosis, or significant bleeding are uncommon but can occur with large envenomations or delayed treatment.

Management focuses on wound care, pain control, and monitoring for coagulopathy. Antivenom, such as CroFab or Anavip, is reserved for moderate to severe cases with significant swelling, systemic symptoms, or lab abnormalities. Most patients recover fully with supportive care, and the mortality rate is less than 1%.
